To register for an online account or place an order with SIGMA, completion of the Test User Qualifications Form is required. This requirement for qualification levels is a standard that was set up in accordance with long-standing guidelines developed by the American Psychological Association. Some assessments require certain levels of training, experience, and/or education before they can be purchased and administered. The purpose is to ensure responsible use and accurate interpretation. We will review your application, determine your qualification level, and respond within 1-3 business days. Required fields are marked by an asterisk (*).
